What Documents are Required to File Bankruptcy?

I request the following documents from my clients in order to prepare their bankruptcy petition:

 * Last six months of pay stubs for each wage earner
 * Proof of any other income received in the past 6 months (including child support, unemployment, worker’s compensation, rental income, pension, social security, etc.)
 * Last 2 years of state and federal returns, including W-2s and 1099s
 * Recent bill or collection notice from each creditor including a mortgage statement
 * Credit reports: http://www.annualcreditreport.com (free)
 * 401K, pension plan or IRA statement showing the amount in each account
 * Current statement of the cash value of any life insurance policy
 * A copy of any divorce decree or property settlement
 * Any court papers, including summons and judgments
 * Copies of any garnishments or liens
